At Bethany House we aspire to support women, and women with children, through transition to
stability. We begin by providing shelter, and meeting the basic needs of our guests, in a welcoming
and safe, home-like setting. The successful candidate will be a self-starting individual who demonstrates the desire and capability to work in a flexible and diverse atmosphere, interacting with a variety of persons from different socioeconomic and ethnic backgrounds. The Case Manager must have the ability to lead a cooperative team in determining and achieving goals, possess organizational and management skills, and have a comprehensive understanding of the complexities of the crisis of homelessness. The candidate should have professional education equivalent to an MSW. Basic knowledge of computer word processing [Microsoft Word, Excel] is essential. Main responsibilities include:
Coordination of screening, interview procedures, assessment and follow-up appointment scheduling for all residents.
Conducting initial Psycho-social Assessment on all residents and periodic review of these assessments to check for accuracy and to add new information as appropriate.
Creating, maintaining, and updating case files on all residents frequently, at a minimum weekly.
Planning, preparation, and conducting of weekly Case Management meetings
Coordination of contacts with agencies for referral, information sharing, etc.
Coordination of welcoming and orientation of new residents.
Coordination of resident participation in case planning and reviews, decision-making regarding case management issues, housing terminations, extensions, etc.
